Core Health Provider Services, Inc has an employment opportunity for a LPN in Marietta, GA 30067. The LPN provides direct nursing care to individuals with intellectual and developmental disabilities (IDD) in accordance with DBHDD standards. This includes administering medications, monitoring health conditions, supporting behavioral health needs, and collaborating with interdisciplinary teams. We are looking for individuals who have a sincere desire to help others. Our mission is to serve adults with intellectual/ developmental disabilities, by providing specialized services. Supports are individually planned and tailored to meet the specific needs of the individuals we serve.
Position Details:
Days: Monday, Tuesday, Wednesday, Thursday, Friday
Time: 8 hour shifts
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Administer prescribed medications and treatments
- Communicates with RN, DDP, and interdisciplinary team as needed
- Participates in team meetings and health reviews
- Documents all communication with physicians and family
- Monitor and document changes in individuals’ physical and behavioral health
- Measure and record vital signs (temperature, blood pressure, pulse, respiration)
- Assist with personal hygiene, mobility, and daily living activities
- Collaborate with case managers, physicians, and behavioral health professionals
- Participate in Individual Support Plan (ISP) meetings and implement care plans
- Maintain accurate and timely documentation in compliance with DBHDD protocols
- Respond to emergencies and adverse reactions appropriately
- Educate individuals and caregivers on health maintenance and medication adherence
- Administers medications per physician orders and documents in MAR
- Monitors and documents vital signs, health status, and behavioral changes
- Provides wound care, catheter care, and other delegated nursing tasks
- Reports significant health changes to RN or physician promptly
- Participates in ISP development and implementation

Required Qualifications:
The successful candidate must:
- Be at least 21 years of age
- At least one year of nursing experience
- Graduate of an approved professional nursing program
- Have dependable transportation, valid driver’s license, and valid car insurance
- Be able to satisfactorily pass a criminal background check
- Provide a clean 7 year driving record, with no suspensions due to moving violations in past 3 years
- Must obtain a TB test
- Current CPR/First Aid certification
- Current physical
- Current Georgia license to practice as a Licensed Practical Nurse (LPN)
Salary: $32.00 - $35.00 /hour
Education:
- Current LPN licensure without restrictions or provisions in the state of GA (Required)
- Completion of DBHDD-required orientation and annual training
- CPR/First Aid certification (current)
- HIPAA and infection control training completed
Compensation: $32.00 - $35.00 per hour
